We are scheduling annual health exams. Currently our schedule is booked until December 2021. We are making every effort to get our patients in as soon as possible. We have an ever revolving cancellation list and we will be happy to place you on it. However, we cannot guarantee that an appointment will become available. Please bear with us as we will are operating well below 50% capacity to ensure the safety of both our patients and staff as this is of the utmost importance.
We are constantly monitoring the CDC guidelines daily as well as local and state guidelines and per those guidelines, have been given the go ahead to reopen for routine as well as medical eye care.
We are requiring all of our patients to wear masks at their visit and if you have been experiencing any of the following symptoms we will reschedule your appointment for a later date when you have been symptom free for 2+ weeks:
We will only allow one person accompany the patient at the time of the exam. The only person in the exam room with the doctor will be the patient unless the patient is a minor under the age of 12.
Another requirement we have put in place is to limit the amount of physical contact between our staff and patients is the use of our patient portal. Prior to your visit, please head on over to the portal and complete your health history. This allows for more accurate record keeping as well as efficiency. Patient portal access - https://kiethjburkartod.com. If you need access and/or currently do not have access, please email us @ Officestaff@kiethjburkartod.com
We are not accepting walk-in patients. To limit the number of patients in the office, we are requiring appointments to pick up your glasses or contacts. Please call (909) 337-4310 or email us to set this up. You may also email us through your patient portal. We are also willing to mail you your glasses or contacts as well.
We apologize for how inconvenient these measures are but hope that by implementing these steps, it limits the spread of the virus in our community.
We will continue to update this page as new information is presented.
Thank you from Dr. Burkart and staff
